PONV History Classification

Description

This phenotype determines if the case has preoperative documentation to support a history of postoperative nausea and/or vomiting (PONV). Collation mapping is performed to categorize raw text values found in the electronic medical record and mapped to specified preoperative MPOG concepts (see logic section for more information). Collation mapping is performed at the MPOG Coordinating Center.

Limitation

Collation mapping relies on both predictive analytics and manual mapping of raw values to appropriate categories. Only variables mapped to designated concepts and including key words such as nausea, PONV, or vomit will be assessed. The entire record is not reviewed to determine history of PONV.

Data quality is limited to individual site documentation accuracy, completeness, and variable mapping, which is performed by each site, independently. MPOG, as the coordinating center, reviews data quality at each data upload and monthly via Data Diagnostics and Case Validation tools. This review, while thorough, may not contain all data elements relevant to this phenotype and thus should be carefully reviewed by the user.

This phenotype creates a list of notes based on keywords found in the values associated with variables mapped to specific MPOG concepts. Those notes are then collation mapped at the MPOG Coordinating Center using predictive analytics and then validated by an MPOG internal reviewer. The list of notes for mapping is created using the following steps:

The collation adds raw text values for variables mapped to MPOG concept 70338 General - PONV Risk Factors
Next, the collation adds raw text values for variables mapped to any of the following concepts that include the keyword 'PONV':
1. 70050 General – Family History of Anesthetic Problems
2. 70080 General – Previous Anesthetic Problem
3. 70099 Assessment and Plan – Special Anesthesia Techniques planned
4. 70102 Misc – Motion Sickness

If the case has multiple values assigned for a single case, the following rules are applied:

If History of PONV (1) is included, assign 1: History of PONV.
If History of PONV in relatives (2) is included (without 1), assign 2: History of PONV in Relatives.
If No history of PONV (0) is included (without 1 or 2), assign 0: No History of PONV.
